Appointments:
You can expect your appointment to begin promptly. Standard therapy appointments are 50-60 minutes in length.
If cancellation is necessary you need to provide at least 24 hours advance notice. A late cancellation fee of $100.00 is charged if less than 24 hour notice is given or a client does not show for an appointment. Your insurance company can’t be billed for missed or late cancelled appointments and you are financially responsible for this fee.
Attendance Policy:
Consistency is important to making progress in therapy and frequently missing appointments is going to hinder or prevent progress. If excessive absence is observed or you have three (3) incidents of late cancellation or no call/no show to scheduled appointments in a rolling 12 month period your therapy at this office may be discontinued.
Telephone Answering:
The office does not have an assistant to answer telephone calls. If your call is answered by voicemail, please leave your name, telephone number and a brief message. I will return your call as soon as possible. If your call is not immediately answered or it is after hours and your call is an emergency, please call 911 or go to the nearest emergency room.
Confidentiality:
Records of the identity, prognosis or treatment of any client are confidential and will be disclosed only with the written consent of the client, the client’s legal guardian, by order of the court of competent jurisdiction, or as otherwise required by law. The office adheres to the Health Information Portability and Accountability Act (HIPAA). In addition, to protect your confidentiality, I will not accept any invitation through social media.
Supervision:
Children 12 and under may not be left unsupervised in the waiting room. As a courtesy to other businesses in the office please make alternative arrangements for children who are not scheduled to receive services at the office. Patients, parents or legal guardians will be financially responsible for any property damage at the office.
Waiting Room Etiquette:
We ask you to be mindful that staff are conducting business and loud noise makes it difficult for staff as well as for our other patients. Please monitor the sound on your electronic devices to minimize disruption to others. You are welcome to bring a drink to the office but please do not bring any food into the waiting room or your session.
Ethical Practice:
I routinely engage in case consultation both as part of maintaining an ethical practice and to meet the requirements of State Law, and in such professional consultations your case may be discussed and all reasonable precautions will be taken to maintain the highest level of confidentiality throughout that process.
Medical Billing:
I have contracted with a medical billing company that submits all claims to insurance. At the onset of services,
client insurance benefits will be verified. It is the client’s responsibility to advise this office of any change in coverage. The billing company will mail account statements at the beginning of each month.
Inclement Weather Policy:
If Omaha Public Schools closes due to inclement weather, our office will be closed for the safety of our
therapists and clients.
